Otoplasty (Greek οὖς, oûs, “ear” + πλάσσειν plássein, “to shape”) denotes the surgical and non-surgical procedures for correcting the deformities and defects of the pinna (external ear), and for reconstructing a defective, or deformed, or absent external ear, consequent to congenital conditions (e.g. microtia, anotia, etc.) and trauma (blunt, penetrating, or blast).[1] The otoplastic surgeon corrects the defect or deformity by creating an external ear that is of natural proportions, contour, and appearance, usually achieved by the reshaping, the moving, and the augmenting of the cartilaginous support framework of the pinna. Moreover, the occurrence of congenital ear deformities occasionally overlaps with other medical conditions (e.g. Treacher Collins syndrome and hemifacial microsomia).

La otoplastia es una cirugía en la cual se resuelven las deformidades de la oreja; estas pueden ser muy variadas desde la ausencia total de la oreja (microtia) a simplemente una proyección muy notable de la misma por la ausencia del antehelix o el helix.
La otoplastia más frecuente es la operación de orejas despegadas (separadas, prominentes, de soplillo, en asa). En los métodos tradicionales (los más conocidos son los de Converse,1 Stenström2 y Pitanguy3 ), el cirujano realiza una incisión detrás de la oreja para exponer y remodelar el cartílago. Ahora existen nuevas técnicas para la otoplastia sin incisiones y simplemente se utilizan suturas permanentes que se colocan tras la oreja bajo la piel para ayudar a mantener la nueva forma de la oreja en su lugar. Entre los métodos más conocidos están el método de Fritsch4 5 y el método de Merck.6
Este procedimiento quirúrgico no mejora la audición del paciente, pero si su autoestima. En los casos de microtia en ocasiones es necesaria la reconstrucción de la misma, la cirugía plástica ofrece muchas alternativas, una de ellas es la técnica de Brent que consiste en cuatro tiempos quirúrgicos y reconstruye la oreja en un periodo de 2 años tras un gran trabajo quirúrgico y artesanal. L’otoplastie est la chirurgie de correction des déformations multiples et variées des oreilles. Celles-ci vont de l’absence totale du pavillon de l’oreille jusqu’à un pli de l’anthélix voire un bord de l’hélix insuffisamment formé.
L’otoplastie la plus fréquente est l’opération des oreilles décollées (oreilles en feuilles de chou, oreilles chou fleur). Lors de méthodes traditionnelles – les plus connues étant celles de Converse1 , Stenström2 et Pitanguy3 – le chirurgien pratique une incision cutanée rétro-auriculaire pour mettre en évidence le cartilage, puis incise ou excise minutieusement le cartilage, et conclut généralement l’opération en retirant une bande de peau sur la partie arrière de l’oreille4. Mustardé met également le cartilage en évidence en incisant derrière l’oreille. Il retire ensuite une bande de peau mais il ne travaille toutefois pas le cartilage5,6. Cette technique avait déjà été découverte auparavant par Morestin7.
Il existe aujourd’hui de nouvelles techniques d’otoplastie sans incisions, qui consistent à positionner des fils permanents sous la peau afin de modeler les oreilles c’est-à-dire leur donner la position souhaitée. Les plus connues sont, entre autres, la méthode Fritsch8 et la méthode Merck9.
L’otoplastie est pratiquée au plus tôt à l’âge de 5 ans, afin d’éviter aux enfants d’avoir à subir des moqueries, et est également très souvent souhaitée par les adultes.
